Financial Medical Reimbursement Applications will be accepted beginning August 1, 2024 until funds have been exhausted

Applications are reviewed as they are submitted and awarded on a funds-availability basis. We receive more requests than we're able to fund, so to increase chances of funding please read the eligibility criteria carefully and submit a complete application.

MUST PROVIDE ALL RELEVANT INFORMATION AND UPLOAD RECEIPTS.

All PAID receipts must be dated between June 1, 2024 and after.

Any receipts dated prior to June 1, 2024 will not be accepted.

Once an application is submitted, no edits will be allowed.

You will receive the status of your application within 6 weeks.
