CC4C serves children between the ages of 5-18 with rare, acute, or hard to diagnose condition which has been either pre-existing for a significant period of time, or recently discovered and expected to continue for an indefinite period.

CHAMPION CHILDREN

Our champion children are the entire reason our organization exists! Each two year season, we recruit and accept 10 children we focus on and pair them up with a Champion Teammate and High School Team who, together, create a team of inspiration and support as the child races toward their recovery finish line.

HIGH SCHOOL TEAMS

The team sponsors a child for the school year, providing special opportunities for the child to feel a part of a team. This program aims to inspire the student athletes to be role models to younger children, and to live and train with care and concern for others.

FAMILY SERVICES

Our Family Services programs are centered on providing support to our families by Family Services Liaisons. CC4C forms alliances with top medical and research institutions to assist our children towards physical wellness. Bringing awareness of the needs of this population, building a body of evidence for supporting families affected, and encouraging research funding.

FAMILY SERVICES LIAISONS

Provide Bi-weekly contacts to check on the status of the CC4C children and identify ways to support the family and child.

EPATIENT FINDER

Holds a database for all clinical trials going on around the world that doctors may not be aware of or have the time to research. EpatientFinder will search for any clinical study that may fit the CC4C children.

PICNIC HEALTH

Is a service that will gather and upload medical records to put them into a digital format for future physicians to view. All records will be kept in one place and uploaded automatically. This is an automatic renewal annual subscription. CC4C will reimburse the initial set up costs and first year subscriptions. After the first year, the subscription fee is the family’s responsibility.

UNDIAGNOSED DISEASE NETWORK (UDN)

Available to CC4C families to submit an application to the program. If accepted, the UDN will review the case and allow access to some of the best physicians in the following clinical sites: Harvard Medical School, Baylor College of Medicine, Duke University, Boston Children’s Hospital, National Institute of Health (NIH), Stanford Medical, UCLA and Vanderbilt Medical Center.

NATIONAL ORGANIZATION FOR RARE DISEASES (NORD)

NORD assists with testing and specific travel costs and funding for rare and certain undiagnosed diseases. Many resources are also offered through their website.

MERCY MEDICAL

Assists with arranging and paying for flights to get medical care for child and one adult family member.

EMPOWERMENT SERIES

Educational calls or webinars that will empower participants to take control of their families health with small steps that can be easily implemented. Calls are hosted by health and medical experts or national partner network. Empowerment Series are generally conducted the last Thursday of each month.

MOTHERS BEST FRIEND

The purpose of Mom’s Best Friend is to emotionally support the mothers of CC4C children by matching each of them with a volunteer who will serve as a “best friend”. It is our hope that through friendship, lunch, coffee, conversations, events, texts, calls and emails, the CC4C moms will be encouraged and feel supported during their child’s illness and recovery.
The MBF volunteer will not be a medical specialist and will be glad to direct any medical questions or concerns to the appropriate person within CC4C to help you (i.e. NORD, Picnic Health, UDN, E Patient Finder, Mercy Medical, etc.).
